Summary

Under indirect supervision, works closely with a specific patient population to improve health outcomes through navigation, education, resolution of barriers, and advocacy through the social determinants of health framework. May work with a multidisciplinary team including healthcare providers, mental health providers, and a wide range of social services, as indicated by the patient, care team, and/or needs assessment(s).

Duties and Responsibilities

  1. Based on the needs of the patient, provides navigation across the continuum of care, including screening, diagnostic, treatment, and support services to address all aspects and phases of a specific health condition.
  2. Assists patients with accessing services designed to support range of social determinants of health.
  3. Helps patients navigate and address barriers to care. Serves as an advocate for patients.
  4. Provides patients access to appropriate supports. Provides resources and information to patients and families through credible, evidence-based sources.
  5. Supports patient-led decision-making and communication.
  6. Provides accurate information and support appropriate to the patient with sensitivity to language and cultural preferences.
  7. Establishes, maintains, and updates files, databases, records, reports and/or other documents. Maintains accurate and complete patient files.
  8. Participates in continuing professional development programs as appropriate.
  9. May provide comfort and support to patients.
  10. Performs miscellaneous job-related duties as assigned.

Minimum Job Requirements

  • High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Knowledge, Skills and Abilities Required

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of community agencies and resources.
  • Ability to effectively interpret needs and act as an advocate for a specified client population.
  • Knowledge of available patient support programs, agencies, and services.
  • Knowledge of social determinants of health.
  • Skill in the use of personal computers and related software applications.
  • Analytical and problem solving skills.
  • Ability to communicate complex information in a manner easily understood by patients.
  • Ability to work both independently and in a team environment.
  • Knowledge of advocacy principles and processes.
  • Skill in accessing internet information services.

Conditions of Employment

  • Possession of a valid New Mexico driver's license is a requirement for this job.
  • Must pass a pre-employment criminal background check.
  • Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.
  • Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.

Working Conditions and Physical Effort

  • No or very limited physical effort required.
  • No or very limited exposure to physical risk.
  • Work is normally performed in a typical interior/office work environment.
