Summary

Abstracts specific patient information from a range of sources, including medical records and interviews with medical staff and/or patients; codes this information into a database according to prescribed protocol, maintaining and editing data for accuracy and completeness.

Duties and Responsibilities

  1. Identifies and compiles patient data from a range of relevant sources, including medical facilities and patients, and arranges for data collection as appropriate.
  2. Abstracts data, including patient demographics, diagnostic procedures, history and extent of disease, treatment, follow-up, physician, and related information from medical records; codes data into database according to strict protocol requirements.
  3. Reviews data for completeness and accuracy and follows up with data sources as necessary.
  4. Researches medical and other relevant facilities and identifies cases for special projects and studies, reviews data for accuracy and completeness, and enters information into database.
  5. Tracks relevant patients and collects follow-up data as appropriate.
  6. Travels to field sites to collect and record data and/or samples as appropriate to the specific objectives of the study.
  7. Performs miscellaneous job-related duties as assigned.

Minimum Job Requirements

  • High school diploma or GED; at least 2 years of experience directly related to the duties and responsibilities specified.
  • Registered Health Information Administrator (RHIA) OR Registered Health Information Technology (RHIT) OR Associates degree in any field with two college level semesters of Human Anatomy and/or Physiology.

Knowledge, Skills and Abilities Required

  • Ability to understand, analyze and interpret medical/forensic information and terminology.
  • Ability to gather data, compile information, and prepare reports.
  • Knowledge of medical data collection codes.
  • Ability to verify data input and correct errors.
  • Basic data entry and/or word processing skills.
  • Knowledge of patient care charts and patient histories.
  • Strong word processing and computer data entry skills.
  • Ability to travel extensively by car to various outreach locations.

Conditions of Employment

  • This position is represented by a labor union and is subject to the terms and conditions of the CWA Collective Bargaining Agreement.

Working Conditions and Physical Effort

  • No or very limited physical effort required.
  • No or very limited exposure to physical risk.
  • Work is normally performed in a typical interior/office work environment.
